With more than 30 years of experience in population health, primary care, and functional medicine, Rewa Banks, DNP, APRN, is dedicated to helping patients achieve sustainable weight loss, hormone balance, and overall wellness. Serving patients in Lebanon and Greater Cincinnati, she provides holistic, patient-centered care focused on long-term results.
Rewa specializes in medical weight loss programs, hormone optimization, and lifestyle medicine, addressing root causes rather than symptoms. Her approach combines medical expertise with compassionate support, helping patients restore energy, improve confidence, and build healthy habits that last.
She earned her Doctor of Nursing Practice degree from Xavier University and has advanced training in women’s health, functional medicine, weight management, hormone therapy. Over the course of her career, she has served in leadership roles where she has been recognized for her ability to engage both staff and patients in collaborative care.
Passionate about health equity and expanding access to care, Rewa ensures that individuals from all backgrounds have the tools and resources they need for healthier, more fulfilling lives.
Outside of her clinical work, Rewa enjoys sport, dogs, gardening, travel and spending time with family. She is also actively involved in community wellness initiatives, which allows her to connect with patients beyond the exam room.

Dr. Chauncy L. Eakins, MD, FAAPMR, DABPM, MS is a board-certified physician specializing in physical medicine and rehabilitation with a focus on improving function through traditional and functional care models. Serving patients in Lebanon, Warren County, and the Greater Cincinnati region, Dr. Eakins is known for his comprehensive approach that blends evidence-based medicine with personalized wellness strategies.
As a Fellow of the American Academy of Physical Medicine and Rehabilitation (FAAPMR), Dr. Eakins focuses on helping patients restore function, reduce pain, and improve longevity through regenerative methods including platelet rich plasma (PRP) injections, bone marrow or mesenchymal cell therapy, exosomes, hormone optimization, diet and exercise. Dr Eakins also provides patient education on peptide therapy. He integrates concierge-style care, offering individualized support, regular check-ins, and advanced therapies to empower patients in their health journey.
Dr. Eakins earned his medical degree from Ross University School of Medicine and completed his residency at Kingsbrook Jewish Medical Center. He has practiced medicine for 18 Years in Practice and maintains active membership in American Board of Physical Medicine and Rehabilitation.
